Weight Loss - Northern Utah
Thank you for your interest in learning more about bariatric/weight loss surgery.
Please note, the best form of communication for our office is through email. Please try to check your email regularly to make sure you don’t miss any messages from us. We use bari1@utahsurgical.com.
Step One: See if you have insurance coverage. You should call your insurance company and ask them if they cover bariatric or weight loss surgery. If they say yes, then continue on. If they say no, we do have cash options available. Also as advocates for access to care, we would recommend that you file a formal complaint with your insurance company and employers that not offering this life saving treatment is discriminatory and they should look at changing their policies. When we approach insurance companies about changing policy, they tell us that patients and employers don’t request it.
ATTENTION: Not all Insurance plans will provide coverage at the facilities where our group provides services.
If you are covered under Blue Cross Blue Shield or United Health Care or any of the following programs that may be offered through your insurance, such as…

- Prime Meridian Health Clinic
Email us with your specific health plan of Blue Cross Blue Shield or United Health Care so that we can verify your coverage before proceeding to step two.
If you do not have Blue Cross Blue Shield or United Health Care, you can proceed to step two.
Step Two: Calculate your BMI.

Insurance Patients:
- BMI under 35 - most insurances will not approve surgery. Surgery may still be an option if you are paying cash and your BMI is close to 35 even if you don’t have any comorbidities (see below). We also offer several medically supervised forms of weight loss including GLP-1 agonists, meal replacements, individual coaching, etc. Email us to request more information.
- BMI over 35 and under 65 - you may qualify for our program. Your insurance will require at least one comorbidity (such as hypertension, diabetes, sleep apnea, arthritis, etc.) for a BMI from 35 to 40.
- BMI over 65 - 80 -Please email us with your current BMI, weight, and height. Note: Weight limit currently for Sleeve/Gastric Bypass is 500 lbs. Weight limit for SADI/Duodenal Switch is 600 lbs.

Step Four : Use the link below to upload the photo side of your driver’s license and both sides of your insurance card(s). This system is HIPAA compliant.
